 General Questions

What can I expect during my first visit?

Dr. Ashlee Richardson, DPT, will meet with you to discuss your concerns, goals, medical history and treatment options. After the initial conversation, an extensive evaluation will be completed to check posture, movement patterns, strength and range of motion. For pelvic floor conditions, a pelvic floor assessment may be performed. After the initial evaluation, treatment will begin and, if necessary, an ongoing treatment plan will be discussed.

How long are physical therapy appointments?

Specific treatment time depends on each patient’s condition and needs. Initial evaluations are approximately 90 minutes and follow-up sessions are approximately 60-90 minutes.

What is pilates-based physical therapy treatment?

Pilates was pioneered by the late Joseph Pilates in the early 1900s as a form of rehabilitation for injured war soldiers, and later for ballet dancers, Broadway performers and actors/actresses. The pilates method emphasizes proper breath patterns, joint stability, postural control and core muscle strength. These principles make pilates and physical therapy a perfect pairing.

Our pilates-based approach gives us better attention to detail and quality of movement education compared with traditional outpatient physical therapy treatment. Combining pilates with treatment also gives patients an opportunity to pair their physical therapy treatment with an ongoing fitness program to aid in the prevention of symptom recurrence and improve overall wellness.

Do I need a doctor’s referral for physical therapy?

No, because Nebraska is a direct access state, a physician’s referral is not required to schedule an appointment. If you would like reimbursement through insurance, we recommend checking with your insurance company prior to your first visit.

Pelvic Floor Therapy Questions

How is a pelvic floor exam performed?

Pelvic floor exams may be done externally and/or internally, based on symptoms and comfort level. Pelvic floor muscles are most often treated intravaginally.

With your consent, Ashlee will first examine the perineal (vaginal) area externally to look for asymmetries, scarring or tissue abnormalities that may help identify areas of inflammation or irritation. This will be followed by an internal exam to assess pelvic floor muscle strength, motor control, endurance and muscle tightness, as well as the ability to relax and contract muscles.

How does a physical therapy internal exam differ from a gynecological exam?

During a physical therapy internal exam, speculums and/or stirrups are not used, and the therapist does not evaluate the reproductive, urinary or intestinal systems. Exams are performed with a gloved finger to evaluate pelvic floor muscles, joints, ligaments and fascia for strength, endurance and motor control.

Ashlee will discuss what she is doing throughout the exam and will be very sensitive to any pain or other issues you may have.

Is an internal exam required for pelvic floor therapy?

If you are uncomfortable having an internal exam on your first visit, or at all, alternative evaluation and treatment options are available. Sometimes patients need time to establish rapport with the therapist or reduce their general pelvic pain to be able to tolerate an internal examination or treatment. An internal exam is still possible while a woman is having her period and is determined by the patient’s comfort level. Internal exams will not be performed if the patient has an active local infection or during pregnancy.
